July 12,  2006

Attendees: Mike Renahan, Jan Buhrman, Sylvia Yeomans, Reid Silva, George Sourati, Elizabeth Pickett-Gray

Pickett House Bed & Breakfast B The Board agreed to issue a Food Establishment permit to the Pickett House after the Building Inspector issues a Certificate of Occupancy.  Ms. Pickett-Gray will also need to obtain confirmation from Nausea Environmental Lab, that the house is fit for human habitation.  This can be done When Dr. Barnes returns from vacation in a couple of weeks.  

Weston Howland III (34-1.3) B The Board approved a disposal works application for a six bedroom residential dwelling at this parcel.

Rosemallow Farm Trust (24-33.2) B The Board granted a conditional approval for this disposal works permit.  They are permitted for eight (8) bedrooms, but there is to be no construction in the barn, a bathroom only.  Any further construction in the barn must be approved by the Board of Health prior to issuing a building permit.  

Meadow Wood Farms, LLC (19-56.2) and (19-56.1) B The Board did not approve these well applications at this time.  They have requested the following information; 1) Why is the owner proposing new wells?   2) What will happen to the existing wells?   3) What is the actual size of the property?  There is a discrepancy between the engineer=s  representation and the assessors= book.  

Lewis & Whitney Sachs (20-21) B This disposal works design is for a shared septic system.  The Board approved a shared septic system for a three bedroom house and a proposed two bedroom guesthouse.  

Julian Wise (11-44) B The Board reviewed the Form C application for this parcel and accepted the engineer=s representation of the soils.  The soils appear to be adequate for a sub division.  

Bart Bussink (7-21) B The Board reviewed the Form C application for this parcel and accepted the engineer=s representation of the soils.  The soils appear to be adequate for a sub division.  

Leah Littlefield (27.1-56) B The Board signed three letters in support of the installation of an alternative septic system manufactured by Wastewater Alternatives of New England, LLC (WANE).  The original letters will be sent to Wesley Brighton at WANE so he can include them in his application package to the Department of Environmental Protection.  Copies will be sent to Sourati Engineering.  

Robert & Francis Labaree (21-36) B The owner has withdrawn this proposed disposal works application.  

Forbes Smiley (20-41) B Sylvia reported that the letters sent certified mail to Mr. Smiley after the last meeting,  have not been accepted and receipts have not been received.  The Board has instructed Sylvia to research Title 5 and find out how to serve the order letter to Mr. Smiley.

David & Karen Davis (24-196, 198, 200) B The Board reviewed a sample maintenance contract from Clivus New England, and a letter from the owners explaining how they plan to dispose of the liquid waste from this composting toilet.  The Board is not certain that the disposal method as stated in the letter meets Title 5 regulations. Sylvia has been asked to research Title 5 for clarification.  The Board has conditionally approved this disposal works application for a composting toilet to serve a five (5) bedroom house.

Marguerite Cottle (24-163.4) B The Board approved a well permit application for this parcel.  The well must be 30 feet from the property line and 30 feet from the easement.

The Board requested that Sylvia send a memo to Tim Carroll explaining that the Town of Chilmark must pay for the phone line and cable lines at the transfer station.



Board of Health Minutes
July 12, 2006


The Board requested that Sylvia send a memo to Tim Carroll asking him to inform Brent Bandi that the Menemsha restrooms need to be pumped out July 1 and August 1.  The marine tank must be pumped as well.

There is some concern regarding the trash removal at Menemsha Park.  Mike asked Sylvia to contact Katie and find out what her thoughts are regarding this issue.

The Board has requested that water samples be taken during the summer months in Menemsha at the following locations; Larsen=s. Menemsha Seafood, The Galley, and under the bridge behind the charter boats.  

A septic installers permit renewal was approved for Brickstone Construction, Inc.

The following tent permits were approved:
Jan Buhrman (12-68)             Joan Wilson (18-117)            Ellen Muglia (17-38.2)

The following sewage disposal system inspection report was received:
Leavy Family Realty Trust (25-77) -  This four bedroom house with a 1,500 gallon septic tank and a      leaching pit passed an inspection done by Reid Silva.  

The following Aas-built@ letter was received:   
Barbara Elias (21-25) - This one bedroom guest house was installed by Vineyard Engineering as per the site plan designed by Reid Silva, and was inspected by John Powers.

The following invoices were submitted and approved:

MV Community Services Visiting Nurse Service                          412.50
